If you are a new patient to our office, this attached file contains our new patient bundle with forms that must be filled out when you arrive. Printing them, filling them out, and bringing them with you will allow us to attend to your medical needs more quickly than completing them upon arrival. We accept several dental insurance plans. And if we do not participate as providers for your specific dental plan, you are welcome to receive your care at our practice. We will assist you in completing insurance forms.
